Cannot be used in conjunction with another dental plan, dental insurance, or financing program such as CareCredit or other third party health care financing. Membership excludes:

• Cleaning in the presence of periodontal disease*
• For referrals to specialists
• For hospitalization or hospital charges of any kind.
• Products purchased through the practice including, but not limited to, whitening gels, toothpastes, rinses, or toothbrushes.
